Minimum Qualifications
- Minimum formal education to include completion of four years of high school, diploma or GED
- Minimum of one year work experience in a related field or volunteer work in related field
- Valid driver’s license and proof of insurance
- Must pass Illinois Activity Professionals Certification process
- Basic computer skills including knowledge of Point Click Care software, Microsoft Office software and ability to navigate internet search engines
Desired Qualifications
- Desired educational abilities equivalent to knowledge, skill and mental development of two years college
- Excellent written and verbal narrative communication skills
- A self-motivated individual, willing to take initiative to implement new ideas to take ownership of position responsibilities
Job Summary
Under administrative supervision, plans, schedules and carries out a complete program of activities. Programs activities of a recreational, diversional, religious, intellectual and service or “work-oriented” nature that meets the individual resident’s needs.
Job Description
- Accountable for supervision of planning and implementation of a well-balanced activity program including (but not limited to) arts and crafts, recreation, religious activities, service projects and trips into the community.
- Interacts with other services to assess and evaluate patient needs, capabilities and response to their participation in activities programs. Develops care plans and appropriate individual activity participation goals.
- Escorts residents to and from special activity areas, to include off campus trips, observing safety precautions and assists in providing for the personal needs and control of residents while participating in all aspects of the activity program.
- Makes observations of residents’ behaviors and informs nursing staff of significant changes in their behavior, reactions or progress.
- Prepares materials and makes appropriate preparations for activities.
- Is responsible for the care and maintenance of supplies, equipment and activity facilities.
- Maintains accurate daily attendance records, charts monthly progress notes, documenting meaningful residents’ responses to activity program, and determine goals for each individual.
- Participates in inservice training programs as a teacher and learner. Attends educational programs outside the facility as assigned, including 10 CEUs annually in an ongoing basis.
- Performs other duties as special circumstances demand.
- Supervises Activity Assistants.
- Supervises a volunteer program which supplements the activity program.
