Social Service Director

SHC is currently hiring for a  Social Service Director. The Social Service Director provides clinical guidance and direct supervision of the Social Service Department. As a Social Service director you will guide and direct the Case Managers, PRSD, Social Service Assistants.

Position Qualifications and Requirements:
  • Have a Bachelor Degree in a Social Service Field, experience in Long Term Care, and 2 years of supervisory experience.
  • Have knowledge of long term care,  psychosocial programming, and activities which enhance resident function and independence
  • Maintain composure in stressful situations and effectively deal with family members, visitors, community providers and personnel from regulatory and accrediting organizations
  • Have the ability to effectively direct and supervise personnel
  • Possess basic knowledge of the facility’s computer systems and programs
  • Maintain a personal schedule which can accommodate emergency situations and any other need for extended or altered hours
Responsibilities:
  • Maintain compliance with practice in accordance with clinical guidelines, governmental regulations
  • Oversee the assessment and evaluation of the ongoing resident psychosocial rehabilitation and social service needs
  • Ensure social service members are hired, appropriately trained, and evaluated in accordance with accepted standards of care and facility policies
  • Ensure adequate staffing to meet residents’ psychiatric/social service rehabilitation needs
  • Developing, reviewing, modifying, and helping implement all components of the Individual Care Plan
  • Ensuring that residents are engaged in discharge planning, as well as linkage to community resources and providers as appropriate
  • Ensure timely and accurate completion of residents’ clinical records and related files. These include, but are not limited to: assessments, treatment plans, progress notes, incident charting and follow-up, identified offender status, court related matters, discharge planning, guardianship, demographic information, group and activity participation, response to medication changes, and room change notification
  • Consult and assist in the resident Grievance Procedure
  • Oversee disciplinary actions for the Social Service department as needed
  • Ensure that social service team members are aware of and in compliance with facility policies and State regulatory requirements
  • Communicate any concerns related to resident status to the appropriate supervisor or interdisciplinary team member
  • Function as the Resident Advocate
